Charnholt Ossuar

In the Bone City the honored dead are continued, folded into the Weave — but sometimes the folding fails, and something rises instead: a Large ossuar, a striding cathedral of fused bone and pale bracket-fungus that remembers, dimly, that it was once a person, and hates the living for it.

Traits

Bonemeld Regeneration. The ossuar regains 10 Hit Points at the start of each of its turns. If it takes Radiant damage, this trait doesn't function at the start of its next turn.

Charnel Presence. Emanation. A creature that starts its turn in a 10-foot Emanation around the ossuar and can hear it has Disadvantage on saving throws against being Frightened until the start of its next turn.

Actions

Multiattack. The ossuar makes two Bone Cleaver attacks and one Marrow Grasp attack.

Bone Cleaver. Melee Attack Roll: +9, reach 10 ft. Hit: 23 (4d8 + 5) Slashing damage plus 7 (2d6) Necrotic damage.

Marrow Grasp. Melee Attack Roll: +9, reach 5 ft. Hit: 12 (2d6 + 5) Bludgeoning damage, and the target has the Grappled condition (escape DC 17). Until the grapple ends, the target's Hit Point maximum decreases by 7 (2d6) at the start of each of the ossuar's turns, and the ossuar regains that many Hit Points.

Bone Storm (Recharge 5–6). Dexterity Saving Throw: DC 17, each creature in a 20-foot Emanation. Failure: 27 (6d8) Piercing damage as splinters of bone erupt from the floor. Success: Half damage only.
