The Choir-Drowned

The Mourning Glass is a black mirror-lake that shows you the drowned dead. On the worst nights it shows you them rising: a Huge legion of the lake's lost, poured into one grieving shape of cold water and pale reaching hands, come to gather more voices for its chorus. It is not cruel. It only wants you to stop being alone.

Traits

Legendary Resistance (3/Day, or 4/Day in Lair). If the Choir-Drowned fails a saving throw, it can choose to succeed instead.

Watery Form. The Choir-Drowned can move through a space as narrow as 1 inch without squeezing, and it can move through other creatures' spaces (Difficult Terrain for it). A creature that ends its turn in the Choir-Drowned's space takes 9 (2d8) Cold damage.

Grieving Presence. Emanation. A creature that starts its turn in a 20-foot Emanation around the Choir-Drowned and can see and hear it makes a Wisdom Saving Throw: DC 19. Failure: The target has the Frightened condition until the start of its next turn.

Actions

Multiattack. The Choir-Drowned makes three Drowning Grasp attacks.

Drowning Grasp. Melee Attack Roll: +12, reach 10 ft. Hit: 24 (4d8 + 6) Cold damage plus 13 (3d8) Necrotic damage. If the target is Large or smaller, it has the Grappled condition (escape DC 19).

Mourning Wail (Recharge 5–6). Wisdom Saving Throw: DC 19, each creature in a 30-foot Cone. Failure: 45 (10d8) Psychic damage, and the target has the Frightened condition (save ends at end of turn). Success: Half damage only.

Claim the Drowned. The Choir-Drowned targets one creature with 0 Hit Points that it is Grappling; that creature must succeed on a DC 19 Constitution saving throw or die, and its voice joins the chorus. The Choir-Drowned regains 20 Hit Points.

Legendary Actions

Legendary Action Uses. 3 (4 when in its lair). Immediately after another creature's turn, the Choir-Drowned can expend a use to take one of the following actions. It regains all expended uses at the start of each of its turns.

Rising Tide. The Choir-Drowned moves up to its Speed without provoking Opportunity Attacks.

Reaching Grasp. The Choir-Drowned makes one Drowning Grasp attack.

Undertow (Costs 2 Actions). Constitution Saving Throw: DC 19, one creature the Choir-Drowned can see within 30 feet. Failure: The target is pulled up to 20 feet toward the Choir-Drowned and has the Grappled condition (escape DC 19); while Grappled this way, the target is Suffocating and takes 9 (2d8) Cold damage at the start of each of its turns.
